  • 2017 Summer Mailing Forms - Update 8/10/17

    Please see the Connect-Ed message that was sent to families on the morning of July 18 regarding the new process for the 2017 Summer Mailing Forms. Please select "Read More" for updates.

    SUBMISSION OF PHYSICAL: You may now upload an electronic copy of a doctor-completed physical form within the Physicians section of the Summer Mailing Forms. If you have already completed your other forms and would now like to upload a physical, please log in to view your summary and select Edit from the drop-down. You may also send it in hard copy to the child's school, or email it to the school's klschools address located in the Contact Information. You do not need to provide it here if it has already been submitted in a 2017-18 Fall Athletics registration.

    STUDENT NAME NOT APPEARING: your student's name will be pre-populated as a participant choice only if he/she has previously participated in HS or MS athletics.

    SECTION SELECTION (the first data-entry field): please select the SECTION that corresponds to the grade level that this PARTICIPANT will be entering in the Fall. If the school and grade level that you need do not appear, then you may have selected the incorrect program for this PARTICIPANT. Please return to the dashboard and enter the full name of the school into the "I want to register for a program at" field.

    FINDING ANOTHER PROGRAM: if you have finished with your John Jay High School 2017 Summer Mailing Forms submission, and would like to next submit your HS athlete's forms for the Athletics Dept, please  return to the dashboard and enter  John Jay Athletics into the "I want to register for a program at:" field.

    UPLOADING A HEALTH APPRAISAL FORM: if you have completed your child’s summer mailing forms, and would like to go back in to upload his/her Health Appraisal Form (required for grades K, 2, 4, 7, 10 and all new entrants), please use the “View or Edit my Registrations” button to update the existing Participant record for your child.

    Dear KES Families,

    Many years ago, when I first became an administrator, a friend was kind enough to give me a book called, Servant Leadership.  In it, the author, Robert K. Greenleaf, describes an approach to leadership that emphasizes service to the constituencies with whom you have the privilege of working.  It is a philosophy that I have followed throughout my career, which now has spanned forty six years. 

    That being said, it has truly been a pleasure, honor and a privilege to serve the KES community for the past eight years.  I have been fortunate to work and live in such a caring and generous community.  It has been an amazing experience that I will always remember and cherish.

    In the near future, Andrew Selesnick, our School Superintendent, will recommend to our Board of Education that they accept my letter of retirement, effective September 1st, 2017.  Of course, I wanted you to be the first to know. I’ve waited until the summer because I didn’t want to do anything to detract or take away from the wonderful spirit that is a part of the end of every school year.

    My plan is to be here at KES for a good part of the summer working with Cristy Harris to get everything ready for the opening of what I know will be another great year here at KES.  I leave you in the capable and caring hands of Cristy, our teachers and staff!

Summer Mailing Forms are now online

We are happy to announce that this year we are able to provide a more convenient way of completing the annual summer mailing forms, through FamilyID ( FamilyID is a registration platform that provides a user-friendly way to complete forms, and also helps us to be more administratively efficient and environmentally responsible. It is already being used by the KLSD Athletics Department for high school and middle school sports.

Please see the recent ConnectEd notification for more information about FamilyID.

The downloadable forms are provided here should you prefer not to participate in FamilyID. You may scan and send the completed forms to, or send them/drop them off in hard copy form. Please submit the completed forms by Friday, July 28th.
